Teacher Talk Group Meeting Summary Beginning Teacher Reflection Form

Support Coach Wiki click here To complete the online support coach contract form click here. Once the online form is completed, click the "Submit" button and a completed contract will appear. Print the contract and check for accuracy in the information. If the information is correct, sign the contract and fax it to Ralph Evans, Director at (252) 257-0345. If the information is incorrect, return to the online form and make the necessary corrections, print the revised contract, and fax it to the director. Teacher Talk Group Meeting Summary Within a week of each Teacher Talk Group Meeting the support coach is to complete the online meeting summary report. To complete this online report click here. Once the online form is completed, click the "Submit" button and a completed report will appear. Print the report for your records. An email will be automatically submitted to the director. After receiving this email, the director will forwarded a copy to the LEA Representative for your district. The report information will also be collected in a database for use in verifying that each support coach has met his/her requirements to receive their stipend at the end of the school year. Beginning Teacher Reflection Form At the conclusion of each Teacher Talk Group Meeting the beginning teacher is to complete a reflection form regarding the meeting. The teacher may use a printed copy (click here) or he/she may use the online report click here. Once the online form is completed, click the "Submit" button and a confirmation form will appear. Print the report for your records. An email will be automatically submitted to the director. After receiving this email, the director will forwarded a copy to the LEA Representative for your district. The report information will also be collected in a database for use in reviewing the impact of the Teacher Talk Sessions on beginning teachers.
